This is the case of female patient Ph.LM, born in 2007, from San Cha village, Dao San commune, Phong Tho district, Lai Chau province; residing at Doan Ket sub-district, Muong Lat town, Muong Lat district, Thanh Hoa province.
The patient presented with a sore throat. After three days of home treatment without improvement, on August 4th, the patient went to a private clinic and was advised to go to Muong Lat District General Hospital on the same day with symptoms including mild fever, sore throat, difficulty swallowing, red and congested pharyngeal mucosa, swollen and red tonsils on both sides, and numerous white pseudomembranes adhering to the surface of the tonsils.
The patient was diagnosed with suspected diphtheria and was transferred to the Department of Tropical Diseases, Thanh Hoa Provincial General Hospital on the same day.
Thanh Hoa Provincial General Hospital examined the patient and noted the following symptoms: sore throat, no fever, no cough, no difficulty breathing; numerous whitish, shiny, tough, tightly adhering pseudomembranes on the uvula and posterior pharyngeal wall; staining results showed diphtheria bacteria; culture results were positive (+). The patient was diagnosed with diphtheria/8-month pregnancy.

On August 5th, the monitoring team from the Thanh Hoa CDC, along with the district-level and local health systems, discovered 4 additional cases of close contact with the patient exhibiting symptoms of sore throat, and 15 close contacts (F1) of the patient.
Provincial authorities instructed the treatment of the environment using Chloramin B at a concentration of 0.1% active chlorine in the patient's home area and in cases of contact with the patient; directed the Muong Lat District Health Center and the Muong Lat Town Health Station to intensify communication on diphtheria prevention and control measures; the benefits of vaccination and full and timely immunization; and to collect samples from suspected F1 cases and send them for staining and microscopic examination at the Thanh Hoa CDC and the Central Institute of Hygiene and Epidemiology for diagnostic testing.
The Muong Lat District Health Center, in coordination with the Muong Lat Town People's Committee, the Police, and the local health station, conducted epidemiological investigations and surveillance, compiled a list of individuals who had direct contact with the patient; collected samples for testing and sent them to the Thanh Hoa CDC; instructed the Muong Lat Town Health Station on environmental treatment and disinfection in the patient's home and related areas; and directed the transportation of suspected cases to the district general hospital for isolation and treatment.
F1 cases must self-isolate at home and monitor their health for 14 days from the last time they had contact with a confirmed case.
The healthcare system continues to monitor and detect suspected cases, isolate and treat them promptly; trace F1 contacts of patients, compile lists, organize isolation, and monitor their health at home according to regulations.
