MEDLATEC General Hospital has just received a male patient PTH (40 years old, Phu Tho) who came to the clinic with fatigue and sweating. The patient said that about 3 days ago he drank tea containing high levels of pesticide residue.

Mr. PTH is a worker at a tea plantation in Phu Tho. During his break, he drank water from a pre-mixed bottle. Unfortunately, the water he drank contained a pesticide called Chlorferan 240SC.

Upon returning home, Mr. H. experienced burning pain in his back, a feeling of heat all over his body, and profuse sweating. Thinking that these were common symptoms of fatigue, he took vitamin C and paracetamol, combined with rest.

After 3 days, Mr. H. saw that his condition did not improve and his body became increasingly tired, so he decided to go to MEDLATEC General Hospital for examination.

Here, after taking a medical history, performing a clinical examination and performing the necessary paraclinical tests, the doctor concluded that the patient's condition included acute rhabdomyolysis and monitoring for poisoning from the active ingredient Chlorfenapyr in the pesticide Chlorferan 240SC.

Poisoning by the pesticide Chlorferan 240SC is an extremely rare case.

Realizing that this was a dangerous and extremely rare poison, doctors at MEDLATEC General Hospital consulted with doctors at the Poison Control Center, Bach Mai Hospital and quickly transferred the patient for treatment there.

Mr. H's health condition is getting worse and worse, he is gradually losing consciousness, his organ failure is increasing, and his prognosis is high. However, after 3 dialysis sessions, as if by a miracle, the patient gradually regained consciousness and continued to be monitored and treated actively.

Warning of the risk of death from pesticide poisoning

MSc. Dr. Pham Duy Hung - Deputy Head of Internal Medicine Department, Emergency Resuscitation Department, MEDLATEC General Hospital said that the PTH patient's pesticide poisoning had a much more complicated progression than other pesticide poisoning cases that the hospital had received because chlorfenapyr is a very rare type of toxicity.

Although the World Health Organization (WHO) identifies this pesticide as moderately toxic, the mortality rate in poisoned patients is very high (about 70%). Currently, there is no specific antidote for chlorfenapyr poisoning.

Chlorfenapyr poisoning is characterized by slow toxicity and decreased ATP production by inhibiting oxidative phosphorylation in mitochondria, thereby causing severe damage to energy-consuming organs, including the nervous, muscular, and cardiovascular systems.

The active ingredient Chlorfenapyr causes serious damage to many organs in the body.

Dr. Hung added that patients poisoned by chlorfenapyr were admitted to intensive care with the following symptoms:

  • Sweating a lot;
  • Signs of dehydration;
  • Fever;
  • Hyperventilation;
  • Tachycardia;
  • Metabolic acidosis;
  • Acute renal toxicity.

According to doctors' recommendations, people who show signs of suspected poisoning or are exposed to toxic substances that pose a risk of poisoning should immediately go to a medical facility for timely treatment instructions.
