On September 15, Ms. K'H. was admitted to Lam Dong General Hospital with a diagnosis of 38.5 weeks of pregnancy, fetal distress, fetal abnormalities causing polyhydramnios, ruptured membranes, preeclampsia, hypothyroidism, and stable treatment. Therefore, the indication for emergency surgery for the patient was correct. The mother had a cesarean section to deliver a 2.4 kg baby girl, the patient was actively resuscitated and transferred to a higher level for continued treatment.
During the care and monitoring process until September 18, the patient had a fever of 40 degrees Celsius. The on-duty doctor performed a full examination and prescribed medication. The patient responded well to the antipyretic and by late that night, Ms. K'H.'s fever had gone away. By the next morning (September 19), the mother had no more fever, only a cough, and wished to be discharged to take care of her newborn baby who was being treated in Ho Chi Minh City. The hospital discharged the patient and instructed her on post-operative care.
On September 21, the mother's health worsened and she was admitted to Lam Dong General Hospital for the second time. The hospital diagnosed Ms. K'H. with severe dengue fever, fever of 40 degrees Celsius, severe bleeding (abdominal cavity had 4,500ml of dark red, thin fluid), and multiple organ failure (heart failure, liver failure, kidney failure).
During the exploratory emergency surgery, the surgical wound was found to be dry and well healed; there was oozing fluid and blood throughout the abdomen that was difficult to stop; paraclinical examination showed a blood clotting disorder. The patient's clinical condition was consistent with complications of dengue fever. The hospital determined that there were no accidents or complications arising during the first cesarean section.
Lam Dong General Hospital determined that the reception, monitoring, and care of medical staff were appropriate. The medical staff of the unit followed the correct procedures, performed emergency resuscitation, and clearly explained the condition to Ms. K'H's family. Despite seeking advice from higher-level treatment, the patient was admitted to the hospital in severe shock with complications of multiple organ failure due to dengue fever and did not survive.
Regarding the condition of Ms. K'H.'s baby girl who was born by cesarean section, the pediatrician received the patient in the operating room for intensive resuscitation and transfer to a higher level. Currently, the newborn girl's health condition is stable and she has been discharged from the hospital. |
