Decoding the case of high fever for more than 1 year due to having 3 dangerous agents at the same time

A 54-year-old man in Da Nang had a high fever for more than a year. Doctors at the Central Hospital for Tropical Diseases discovered that he had a virus, bacteria, and fungus at the same time.

The Central Hospital for Tropical Diseases has just successfully treated patient TVN (54 years old, Da Nang) who had a high fever that lasted for more than a year without a clear cause. In the past six months, the patient had a continuous fever of 39-40 degrees Celsius, accompanied by headaches, chills, and poor appetite. Antipyretics were ineffective, and his weight dropped from 67 to 49 kg. The patient had visited many hospitals but was at a dead end, before going to Hanoi for treatment on the advice of relatives.

At the Department of General Infection, the patient was admitted in a weak state, with a fever of 39 degrees Celsius and sepsis of unknown cause. Tests detected three agents simultaneously: Epstein-Barr virus in blood and lung fluid; multidrug-resistant Pseudomonas aeruginosa in bronchial fluid; and Aspergillus fungus. CT scan showed lung lesions and enlarged mediastinal lymph nodes.

Dr. Le Van Thieu said: "Each of these agents can cause serious illness, but when combined, they make the condition persistent and life-threatening." The patient was treated with a combination of antibiotics and antifungal drugs according to the antibiotic susceptibility test. After one week, the patient's fever was gone; after two weeks, his weight increased to 54 kg, and his health improved significantly.

According to doctors, cases of persistent fever with difficult to find causes need to be thoroughly examined because the cause can come from many sources such as viruses, bacteria and fungi. Only when the cause is fully determined, treatment will be effective.
